To all Orphanages;: This is to notify you that the present form of application will continue in use for all NEW cases. It is to be filed in duplicate. Attention is called to Yd spaces on the reverse side which are to be filled out by the institution down to space labeled "Why withdrawn". On the face of the blank is to appear on line labeled "Petitioner" the name of parent or guardian of the child. This signature must be affirmed by a Notary or some person connected with the home and empowered to act in that capacity. A new form of renewal blank is ready for distribution end ought to be in this office on July lst of cd ee each year. This is also to be filed in duplicate. Please notify us how many you need. It is to be Signed as the above unless the child is without relatives when a statement to this effect is to be made over the Signature of the Superintendent. In the matter of making the semi-annual claim for state money we beg to advise you that for the period ending June 30th, 1917 the same course as in previous years must be pursued. Thereafter this procedure will be Simplified. Complete details in regard to this will be furn= ished all institutions before the time for filing future clains.
